The process is for the MDBA Breeder/MDBA Rescue of the puppy/dog or the puppy/dog buyer to fill out this form and submit it.

There is no fee for this lifetime membership to the MDBA as only people who have bought a puppy or dog
from a MDBA Breeder or MDBA Rescue can have this membership.

Please be assured that the MDBA treats as confidential all information applicants provide.
